Case management is an individualized service facilitated by staff to insure that the various levels of client needs are met. It consists of face-to-face contact to determine needs, assess resources, develop action plans, and monitor client's progress. Case management implies collaborative and cooperative working relationships with clients in this honest assessment. Case management also implies help in gaining access to resources and making clients aware of their rights and entitlements. Our approach to case management is one of self-help, with guidance and advocacy provided by our staff.
