Volunteer Activity Report
Sustained service volunteers are required to turn in an activity report every two weeks. These reports are due on the Monday following the 1st and the 15th of every month in which the volunteer served.

Tuberculosis Test
Most sites require you to complete a TB test before you can begin serving as a volunteer. We can accept documentation of a TB test from any location, completed within the last two years. When scheduling your TB test, please note that
you must return to the health department 3 days after the initial test to confirm the results. Please keep your reciept for these services and we will reimburse you for your expenses.

LiveScan (Fingerprint background check)
You must complete a LiveScan, even if recently done for someone else, unless your site has specifically exempted you from this process. Please keep your reciept so that we may reimburse you.
Eureka:
Humboldt County Sheriff's Department.
Location: Court House building, 826 4th Street, Eureka.
Hours: Tuesday, Wednesday, and Thursday from 8:00-12:00 and l:00-4:00.
Fee: $20.00 (They only take cash.)
Arcata:
Humboldt State University
